Transition of Care
Complete the Transition of Care Services Request Form
If you are transitioning to Connected Care and would like to continue to receive specifically approved healthcare services from your treating physician (who is not part of the Connected Care network) for a defined period of time (usually 90 days), you must apply for Transition of Care/Continuity of Care.
To apply, complete the Transition of Care Services Request Form. This form is to help you to transition you or your family’s healthcare to Presbyterian Health Plan, Inc./Presbyterian Insurance Company, Inc. You may need to speak with your medical provider to complete sections of this form.
Extra Bucks
Program
If you are currently enrolled in the Anthem Blue Cross High Deductible Health Plan (HDHP) and you enroll in the Connected Care High Deductible Health Plan, any unused Extra Bucks dollars will be transferred.
If you are currently enrolled in the Connected Care HDHP and have an Extra Bucks Account, your funds will continue to be available for you to submit claims for reimbursement for qualified expenses.
